• $400 Deductible ($800 maximum per family)
• $2 Million Maximum Benefit
Dental
• 80% Payment on Most Preventive and Basic Care
• 50% Payment on Most Major Restorative Care

• $400 annual family wellness benefit, no deductable or co-payments.
Vision
• $150 per person plan maximum on eligible charges
Life Insurance
• $5,000 for Each Employee
• $2,500 for Spouse
• $1,000 for Each Dependent
Weekly Disability Income

Instant Vesting
• Beginning With Employee’s First Hour of Work
Defined Contribution
• No Unfunded Liability for the company
Covers All Categories of Work

Contribution Rate
• $1 per hour or Davis-Bacon

The MCA Health Care Program and MCA Retirement Plans are available for both your salaried and hourly employees.

All plans are portable, with the hour bank, within the MCA Health Trust. Eligibility for benefits coverage follows the employee if they move to another participating company in the MCA Health Trust.
