- Annual Deductible-$400/individual, $800 per family
- Co-pays at 80%/20% after meeting deductible for next $10,000 eligible charges; then 100% for the remainder of the calendar year
- Wellness Benefit-$600 per family, no deductible or co-payment
- Well-Child Care Benefit- No deductible/20% co-pay for eligible charges through age 7
- Prescription Drugs-generics paid as 100%, no deductible; Brand- 80%/20% afterdeductible
Dental
- $1,000 Annual Benefit
- Annual Deductible-$100 per individual/$200 per family
- Preventative Care-eligible charges paid at 100%, no deductible
- Basic Care-eligible charges paid at 80% after deductible
- Major Restorative Care-eligible charges paid at 50% after deductible
